The population is rapidly aging and presents new challenges to a healthcare system that is ill-prepared to address the complex needs of older adults. Many healthcare professionals lack specialized training in geriatrics, leading to suboptimal care and potential harm. Furthermore, the overuse and inappropriate prescribing of medications are pervasive issues that contribute to adverse events and cognitive and functional decline.

This presentation will describe the background and goals of the Age-friendly Health Care Systems initiative, developed in 2017. It will focus on the 4Ms framework, which is the basis of this initiative: What Matters, Medication, Mentation, and Mobility.

To improve health outcomes and quality of care for older individuals, patient advocates are among the many stakeholders in this initiative. This framework offers an evidence-based tool for advocacy consultants as they help older adults navigate their health care and successful management of multiple medications and health conditions. This presentation will discuss how advocacy consultants can apply knowledge of the 4Ms and age-friendly care in their practice settings to empower their clients during healthcare encounters.
